The Social Worker engages directly with the patient and their relatives to complete a social history, exploring factors that can facilitate the patient’s smoother adaptation to the Attica Recovery & Rehabilitation Center, with respect for their individuality and supportive network. The Social Worker is responsible for gathering all necessary information and conveying it to the Interdisciplinary Team, ensuring that the social aspects of each case are understood, including the social factors that influenced the problem’s occurrence and any potential future social challenges. However, the primary goal of the Social Worker at the Attica Recovery & Rehabilitation Center is to create a safe environment for the patient and to build trust with the Rehabilitation Team responsible for their recovery. Additionally, the Social Worker must inform and guide the patient regarding welfare benefits and their rights, aiming to secure a good quality of life for them and their family. This may involve accompanying the patient to public services or wherever else is deemed necessary. Lastly, the Social Worker encourages the patient’s active participation in social and cultural activities and events, as well as creative engagement groups, to promote independence and a successful reintegration into society. Furthermore, at the Attica Recovery & Rehabilitation Center, a Social Reintegration Team has been established, consisting of dedicated Healthcare Professionals who collaborate to support the recovery and social reintegration of individuals. This team includes Social Workers, Psychologists, Neuropsychologists, Occupational Therapists, and Speech Therapists. Combining specialized knowledge and skills, the Social Reintegration Team works collaboratively to provide individual support and programs that promote social inclusion after a significant event. Through various activities such as workshops, museum visit groups, and theater events, creative expression, social interaction, and personal development are encouraged. With passion and dedication, the Social Reintegration Team at the Attica Recovery & Rehabilitation Center supports patients, motivating them to discover their potential and build a positive, independent, and fulfilling life.
